Print one for each entry

Press Print this form for the team form on one page of Letter or A4. Each team gets a copy, fills it in by pen and hands it back with the fee.

Forms by sport

The Sheet menu has seven more. Singles and Doubles suit a tennis tournament registration form template. 3 on 3 basketball has four player rows, Golf four with Handicap, Bowling five with Average, Cricket 15, and Petanque four with Singles, Doubles and Triples boxes.

What the forms leave out

No rules and no waiver or consent wording are printed. The foot of each form has a Waiver attached line, for an event that staples its own release to the form.

  • As a soccer tournament registration form template, the team form's 18 rows hold eleven starters and seven more.
  • The short forms print two to a page with a line to cut along, so one sheet of paper serves two entries.
  • For one list that everyone signs on a notice board, print the tournament sign-up sheet.

Questions and answers

Can teams register online with this form?

No. It is paper only: nothing is sent, and what you type on it stays in this browser. Print a copy for each entry, or choose Save as PDF in the print window and send the file.

Is there a free golf tournament registration form template?

Yes. The Golf sheet has four rows for Golfer, Handicap and Signature, two forms to a page. A single golfer or a pair leaves rows empty, so it serves as a golf tournament entry form for any group size up to four.

What is on the 3 on 3 basketball tournament registration form?

Four player rows, for the three players and one substitute of a 3x3 team, with Age, Signature and Parent signature columns. An organizer who collects 3on3 basketball tournament registration forms on paper gets two to a page.

Do you have bowling tournament entry forms?

Yes. The Bowling sheet has five rows for Bowler and Average, and each row has tick boxes for the events that bowler enters: Team, Doubles and Singles.

Which form suits cricket or petanque?

Cricket is a cricket tournament team registration form with 15 rows: eleven players and four more. For a registration form petanque tournament organizers can hand out, the Petanque sheet has four rows with Singles, Doubles and Triples boxes.
