This free soccer sign up sheet opens on 16 numbered player lines. Birth year and age group have boxes of their own, for a league that sorts its teams by age. Print it blank or type the names in first. Nothing you type is sent.

Teams takes one team to a line, for a tournament or a league that enters whole teams: the team, its coach, the age group and a phone number.

A boys and girls soccer sign up sheet

For a club that signs up boys and girls at one table, print a copy for each and write which it is on the top line, beside the season.

Questions and answers

Is this soccer sign up sheet template free to print?

Yes. Print blank sheet prints the numbered lines and the headings with every box empty, on one portrait page of US Letter or A4. There is no account to make.

What do I write under Age group?

The group your league or club gives the player, such as U8 or U10. The sheet prints no table of birth years. If you are not sure, fill in Birth year and leave Age group for the registrar.

Can I add a box for shirt size?

Yes. The plus after the last heading adds a box with an empty heading: type Shirt size into it. The new box prints on the blank sheet too.
