Pitcher, catcher, infield or outfield. A player can write more than one.
Division: your league's own word
T-ball, coach pitch or whatever your league calls its levels goes in the blank on the top line, by pen.
Age: as the league counts it
If the league sets a date that ages are counted from, write the age on that date.

How it works

This baseball sign up sheet opens on 15 numbered player lines, and on paper the top line has a Division blank. Print it for sign-up day, or type the names in first. Nothing you type is sent.

A softball sign up sheet from the same page

No box is baseball only and the printed title reads Baseball or Softball, so the opening sheet is a softball sign up sheet as it stands. Teams is the softball team sign up sheet for an adult league: one team to a line.

Baseball sign up sheets for a school team

School team has Grade in place of Age and no phone box, for the list a coach posts before tryouts.

Questions and answers

Are these free blank baseball sign up sheets?

Yes. Print blank sheet prints the numbered lines and the headings with every box empty, as many copies as you need. Each sheet fits one portrait page of US Letter or A4.

Is there a template for a baseball sign up sheet by division?

This one. On paper the top line has a Division blank: print a copy for each level, so every coach is handed the list for one division.

Is it safe to type a parent's phone number?

Nothing typed is sent to us; it stays in this browser. The printed sheet is the thing to mind: left hanging at the field, it is read by anyone who walks past, so keep it on a clipboard at the sign-up table.
