Guard, forward or center. A player who has not played before leaves the box empty.
Height in feet and inches
Write 5-4 for five feet four.
Shirt size: the jersey to order
The family writes the size as it would order it, youth or adult.
Phone is the parent or guardian
For a youth sign-up, write the number of the adult to call. On a sheet taped to a wall anyone can read it, so an email address is enough.

How it works

This basketball sign up sheet template opens on 15 numbered player lines. Print it blank for the gym door, or type in the names you already have. It is a paper sheet: it takes no sign-ups online, and nothing you type is sent.

A 3 on 3 basketball sign up sheet template

3 on 3 teams puts a whole team on one line, signed in by its captain, so nobody is entered on two teams.

A weekly sheet for open gym

Open gym is a plainer basketball sign up sheet: weekly sessions need only a name, a grade and a phone number.

Questions and answers

Can I save the basketball sign up sheet as a PDF?

Yes. Press Print blank sheet, or Print filled sheet once names are typed, and choose Save as PDF in the print window. Each sheet is laid out for one portrait page of US Letter or A4.

Does it work as a high school basketball sign up sheet?

Yes. Grade already has a box, and any heading can be typed over: Shirt size can become Level for JV or varsity, and Phone can become Email. A heading you type prints on the blank sheet too.

How many players go on a 3 on 3 basketball sign up sheet?

Four to a team: three on the court and one substitute, the squad of the international 3x3 game. Each line has boxes for the team's name, the captain, two more players and the sub.
